Depression Unstuck: Targeted Modulation of Mood and Motor Circuits using DBS

 

Deep Brain Stimulation, the targeted modulation of discrete neural circuits using implanted electrodes, is an emerging experimental treatment strategy for patients with intractable major depression. As testing of DBS in this context has expanded and matured, neuroimaging continues to play a crucial role, with recent work now focused on refinement and optimization of the procedure using device innovations and multimodal methods combined with real-time behavioral and electrophysiological metrics. Together, these studies offer a unique perspective on critical pathways and mechanisms mediating both acute and sustained antidepressant effects of high frequency DBS, and perhaps on the pathophysiology of treatment resistant depression more generally.
